Stem Cells Show Promise as Heart Failure Treatment

HealthDay – 1 hr 40 mins ago MONDAY, July 23 (HealthDay News) -- Scientists who used modified stem cells to rejuvenate damaged and aged heart cells from elderly heart failure patients say their research could one day lead to new treatments for the illness.

"Since patients with heart failure are normally elderly, their cardiac stem cells aren't very healthy," Sadia Mohsin, one of the study authors and a postdoctoral research scholar at San Diego State University's Heart Institute, explained in a news release from the American Heart Association (AHA). "We modified these biopsied stem cells and made them healthier. It is like turning back the clock so these cells can thrive again."

The study was to be presented Monday at an AHA meeting, in New Orleans, and published simultaneously in the Journal of the American College of Cardiology.

The stem cells taken from the patients were modified with a protein called PIM-1, which promotes cells survival and growth. The modified stem cells helped the signaling and structure of the patients' heart cells by boosting the activity of an enzyme called telomerase, which elongates telomeres.

Telomeres are the caps on the ends of chromosomes that are involved in cell replication. Aging and disease occur when telomeres break off.

"There is no doubt that stem cells can be used to counter the aging process of cardiac cells caused by telomere degradation," Mohsin said.

While the study did use human heart cells, it was conducted in the laboratory and remains in its early stages. However, tests conducted in mice and pigs also found that telomere lengthening resulted in new heart tissue growth in just four weeks.

"Modifying aged human cardiac cells from elderly patients adds to the cell's ability to regenerate damaged heart muscle, making stem cell engineering a viable option," Mohsin said. "This is an especially exciting finding for heart failure patients. Right now we can only offer medication, heart transplantation or stem cell therapies with modest regenerative potential, but PIM-1 modification offers a significant advance for clinical treatment."

European regulator: Gene therapy treatment OK

The European Medicines Agency is recommending the first-ever approval of a gene therapy treatment in the EU, in a significant move for a type of treatment that has so far failed to deliver on its promise to cure diseases.

In a statement on Friday, the EMA said Glybera, made by Dutch company uniQure, should be approved across Europe for the treatment of an extremely rare disorder that leaves people unable to digest fat. The treatment consists of a gene that makes a protein to break down fat.

Gene therapy is an experimental technique that tries to cure diseases by replacing genes that don't work. It has never been approved in the U.S. and most trials over the past two decades have failed. China was the first country to approve a gene therapy treatment in 2003 for cancer.

Scientists have struggled to find ways to deliver the genes safely, often by using a harmless virus. There are also concerns that inserting a gene at the wrong spot could cause cancer or that the body's immune system might attack the new gene and the virus used to deliver it.

The EMA previously rejected Glybera three times but it was reconsidered at the request of the European Commission. The agency recommended approval under tough restrictions and will require the company to set up a registry to closely track patients. Previous trials of the treatment only tested it in 27 patients.

"It is only meant for patients with the greatest need," said Monika Benstetter, an EMA spokeswoman, explaining the gene therapy is intended for people with no other treatment options. She said only a handful of gene therapy treatments had been considered before

Arthritis Treatment Linked to Liver Problems in Study

HealthDay – 4 hrs ago MONDAY, June 18 (HealthDay News) -- A "medical food" called Limbrel, which doctors prescribe to treat osteoarthritis of the knee, was linked to several cases of liver disease in a small study, but the effects so far seem to be rare and easily reversible.

Still, patients who take Limbrel, also known as flavocoxid, should be aware of the potential for liver problems, said study lead author Dr. Naga Chalasani, director of the division of gastroenterology and hepatology at Indiana University School of Medicine, in Indianapolis.

Patients should not assume that "medical foods," such as Limbrel, are 100 percent safe, he added.

In the United States, medical foods are not subjected to the clinical trials required of prescription drugs before coming to market.

According to Primus Pharmaceuticals, Inc., maker of Limbrel, the main ingredients of the pills are plant elements known as bioflavonoids, specifically baicalin and catechins. The company says Limbrel helps improve mobility and relieve joint discomfort and stiffness related to arthritis.

For the new study, Chalasani and colleagues analyzed 877 cases of liver injury and found four linked with Limbrel. The researchers said it's "highly likely" that the product caused the liver problems in three of the patients and possible in the other one.

Symptoms included nausea, fatigue and yellow skin, Chalasani said. The four patients were women between 57 and 68 years old who showed signs of liver illness between one and three months after taking Limbrel.

They recovered within weeks of discontinuing the drug.

"Our report provides convincing evidence that flavocoxid is capable of causing clinically apparent, acute liver injury," the study authors wrote. They also noted that the pharmaceutical company has discovered 31 possible cases of liver problems among more than 284,000 users since the drug was brought to market in 2004.

It's not clear why Limbrel might cause liver problems, although the researchers suspect one of its chemicals may be at fault, Chalasani said.

According to Dr. Robert Levy, director of clinical development for Primus Pharmaceuticals, "Limbrel is, by far, the safest anti-inflammatory on the market."

Limbrel has an "extraordinary safety profile," he added. He said medical foods must be prescribed and used under the direction of a physician, adding that physicians should monitor the livers of patients who take the drugs.

"Because the liver is the site of metabolism of most drugs and foreign chemicals, a great many drugs, including all anti-inflammatory agents, are known to have some liver toxicity," Levy said.

Painkillers, another common treatment for osteoarthritis, only provide limited relief and are also associated with adverse events, the study authors said.

About 20 million Americans suffer from osteoarthritis, which is the leading cause of disability in the United States, according to the U.S. National Institutes of Health.

Study lead author Chalasani said patients and physicians should be aware that medical food products haven't gone through the same kind of review as regular prescription drugs.

"I'll bet most people who prescribe Limbrel think it's gone through the typical review process," he said. "If a physician chooses to prescribe Limbrel, he or she should be aware that it can cause this toxicity." If patients develop symptoms of liver damage, they should stop taking Limbrel, he added.

The study appears in the June 19 issue of the Annals of Internal Medicine.

The authors of an accompanying journal editorial said that the current policy of allowing medical foods to come to market without rigorous testing may need to be reconsidered, given their popularity and potential for damage.
